The Denver Hospice is hiring Certified Nursing Assistants to travel between homes and facilities to care for our beloved Hospice Patients.

Openings throughout Denver metro area.
Full time- (Monday- Friday 8-5)
Part time- Weekends

POSITION SUMMARY:
The Certified Nurse Aide (CNA) provides quality nursing care within their scope of practice to promote comfort and dignity for patients in the hospice care setting. Upholds positive public relations with patients, families and facility staff. The Certified Nursing Assistant is able to work with neonates through geriatrics.
KEY REPORTING RELATIONSHIPS:
Reports to the Clinical Manager.
Position may receive supervision and delegation from the Registered Nurse (RN).
ESSENTIAL DUTIES:
Delivers care based on the Plan of Care (POC) established by the RN Case Manager and/or assigned RN. Communicates to Clinical Manager, RN Case Manager or assigned RN regarding their observations and knowledge of patient needs and preferences. Is a part of the Interdisciplinary Team/Group (IDT/IDG) and participates in meetings as assigned.
Assists with ambulation, range-of-motion exercises, positioning, turning and transfers according to patient’s abilities, tolerance and preference.
Takes and records vital signs, observes changes in patient’s condition or behavior; promptly reports any changes to appropriate licensed nursing personnel.
Provides personal care to patients, including bathing, peri-care, shampoo, oral hygiene, denture care, nail care, shaving and assisting with dress.
Aides with patient’s toileting needs, including bedpan/urinal use, catheter care, changing briefs, and when delegated and supervised, administers suppositories/enemas.
Periodically addresses food and fluid issues by preparing meals, feeding patient and/or offering fluids.
Maintains patient’s skin integrity to prevent pressure ulcers, skin tears or other damage by changing incontinent patients, turning, repositioning and applying lotion.
Other duties and responsibilities as assigned.
CLINICAL CORE COMPETENCIES:
Supports the mission and values of hospice and palliative care.
Provides patient care (neonates to geriatrics) as determined by the Plan
Possesses knowledge of patient rights; ensuring an atmosphere which allows for the privacy, dignity and well-being of all patients in a safe and secure environment.
Understands the correlation between excellent patient care and the results of Hospice Item Set, CAHPS and other publicly-reported industry and/or accreditation quality benchmark programs.
Treats patients, family members and other clients with concern and empathy; respects their confidentiality and communicates in a courteous and respectful manner. Maintains and respects professional and personal boundaries. Adheres to HIPAA regulations, and maintains confidentiality of all patients and families.
Performs job duties within the standards of practice framework.
Attends team meetings and in-services on a regular basis. Participates in continuing education and professional development courses, to include annual education requirements as assigned to the discipline.
Works in a high volume environment with rapidly changing priorities.
Provides accurate and timely documentation in Electronic Medical Record (EMR). Uses, clear concise and professional written and verbal communication.
Responsible for meeting visit frequency and visits per shift requirements; takes on-call rotation as needed and/or assigned.
Possesses knowledge of necessary computer systems including Microsoft Office, Outlook and agency electronic medical records (EMR).
Promotes a holistic approach to patient care.
Cooperates with all co-workers; demonstrates flexibility to perform other duties as assigned.
Job Requirements
Hospice experience preferred. If no Hospice experience, we prefer the desire to care for Hospice patients.
Minimum Education: High school diploma or equivalent.
Minimum Experience: 3-6 months CNA experience (hospice preferred).
Required License: Current Colorado CNA license. Valid Colorado driver’s license and proof of auto insura
